NRC chairman points to Peach Bottom example of following all the right steps and and still not reaching the right outcome

...Senator Carpel [and I] just finished a tour of the plant, and talked with Exelon managers about the security issue involving inattentive security officers. Let me say up front that I am not going to make excuses for what happened here. The NRC missed it. The plant missed it. From the NRC perspective, this is an example of how you can follow all the right steps and obey all the appropriate procedures, and still not reach the right outcome. In March of 2007, when the NRC first received the allegation, we responded in what we thought was an appropriate way... We investigated ,,,, but neither we nor the plant managers found evidence of the problem aleged... Clearly, the NRC needs to review the whole process of how we do those inspections.

Summary of Plant Status - April 1, 2005 - June 30, 2005

Both Unit 2 and Unit 3 began the period operating at full power. On May 6, 2005, Unit 2 commenced a power reduction for planned maintenance that included main condenser water box cleaning and a condensate pump motor replacement. Unit 2 returned to full power on May 10. On May 13, 2005, Unit 3 commenced a power reduction for planned summer readiness maintenance that included main condenser waterbox cleaning. Unit 3 returned to full power on May 15, 2005. Both units were at full power for the remainder of the period, except for brief periods during planned testing and rod pattern adjustments.

NRC sends AIT to Peach Bottom re Sept 15 NOUE

NRC Region I has dispatched a six-member Augmented Inspection Team to Peach Bottom to look into the causes of, and the plant response to, the shutdown of the twin reactors at the site on September 15th. An electrical grid disturbance has been identified as the initiating cause of the shutdowns.

Peach Bottom activates E-plan after offsite power interruption and diesel generator problem

At about 1:32 a. m. on Sept. 15th, at a location far from the plant site, breakers on the grid apparently did not function properly to isolate the effects of a lightning strike, resulting in a lowvoltage condition on both of the off-site power lines that feed Peach Bottom. Like other nuclear power plants, Peach Bottom not only sends power to the grid, it also receives energy back for operational purposes. If those power sources are significantly reduced or interrupted, an affected plant will shut down. That is what automatically occurred at Peach Bottom immediately after its off-site power sources were impacted.

As they are designed to do, the plantÕs four emergency diesel generators started upon the interruption of off-site power. Those generators are used to provide power to key safety systems and assist with the safe shutdown of the reactors. At 2:30 a. m., one of the emergency generators tripped, or switched off, because of a problem with its diesel engine coolant system. The plant continued to have sufficient power to safely shut down the reactors. However, following the generator trip, Exelon declared an Unusual Event, based on the shift managerÕs discretion, because of what were perceived to be degrading plant conditions. An Unusual Event is the lowest of four levels of emergency classification used by the NRC.

Exelon restored the normal configuration of its power supplies at approximately 7:40 a. m. The Unusual Event was declared over at 10:46 a. m.
