NRC cites Oconee nuclear plant for inspection finding of low to moderate safety significance
Oconee's fire response procedures were not consistent with the plant's licensing basis for manning of the Standby Shutdown Facility. NRC identified scenarios in which transfer of control to the alternate facility could have been delayed long enough to result in problems meeting NRC fire protection regulations. The plant has already taken corrective action, modifying the procedures. The classification of this violation as "greater than Green" seems an exception to the "no harm, no foul" kind of approach that NRC has used in recent years. May 18, 2004 Oconee has turned around, safety-wise, in last 5 years, says UCS' Lochbaum: "Oconee shows things can be done right" Duke Energy and Oconee have turned their safety records round, said David Lochbaum, a nuclear safety engineer with the Union of Concerned Scientists. The plant was "marginal" five years ago, he said. "Oconee shows things can be done right," Lochbaum said. "It makes us wonder why others aren't doing it." The Oconee Nuclear Station has filed one emergency and nine non-emergency event reports with the NRC since Jan. 2. The most recent came Monday when worker inside the non-operational No. 2 reactor had chest pains and was sent to Oconee Memorial Hospital. The report was filed because the worker might be contaminated. He wasn't. The uptick in reports could be because of a safety culture that looks for problems before they become serious, Lochbaum said. "If you're not finding problems, then things are only going to get worse with time," he said. Duke expects its employees to find and fix problems themselves, said Rose Cummings, a spokeswoman for the company. Duke operates three nuclear plants in the Carolinas: Catawba and McGuire near Charlotte in addition to Oconee. All three have been granted license extensions to operate another 20 years. "We recognize our employees are our best sources of information," she said. "We have a great investment in the Carolinas in these plants. It's our backyard, too." Here's the abstract: "Unit 3 entered its scheduled end-of-cycle 20 refueling outage on April 26, 2003. On May 2, 2003, a' visual inspection of the bare reactor vessel head (RVH) was performed. Results of the visual inspection revealed two (2) control rod drive (CRD) mechanism (CRDM) nozzles that were suspected of leakage. Of these, CRDM No. 4 was observed to contain a very thin white coating on the nozzle and CRDM No. 7 appeared to have a small accumulation of boron on the head adjacent to the annulus region. In addition, approximately 6 to 8 CRDMs could not be visually inspected as they were masked by deposits from a Component Cooling (CC) system leak above the RV head. "Subsequent evaluation of the prior refueling outage RVH inspection videotape showed that the CRDM No. 7 deposits were not associated with a new leak but rather were remnants from a prior refueling outage leak and repair campaign where the boron residue had not been completely removed from the RVH during the wash down process. The CRDM No. 4 boron deposit appeared fresher, exhibited characteristics similar to prior RVH leaks and as such, was conservatively identified as a leak. The apparent root cause of the nozzle leak is primary water stress corrosion cracking. "This RVH will be retired from service and replaced with a new RVH prior to unit restart. For this event, the overall safety significance of this event was minimal and there was no actual impact on the health and safety of the public. " |
