DC Cook wants to allow reactor trip system (RTS) and engineered safety feature actuation system (ESFAS) instrumentation to be tested in bypass. The plant submitted an application about this to NRC, requesting amendment to tech specs, on September 15, 2006. NRC staff reviewed the application, and determined that insufficient information was provided. The following details are from an email from NRC to Cook, proposing that a conference call be scheduled for Cook to discuss the issues with NRC reviewer Hukam Garg.

The most notable omission, NRC told Cook, was the lack of information on the changes to the RTS and ESFAS systems to include the bypass capability. Page 5 of the submittal, titled Technical Analysis, states that: "Hardware changes necessary to be made to the NIS and Foxboro analog/digital protection system to facilitate testing in bypass will be implemented in accordance with 10 CFR 50.59." Standard Review Plan, Chapter 7, states that the review should include an evaluation of the protection system design against the requirements of ANSI/IEEE Std 279, or Reg. Guide 1.153, which endorses IEEE Std. 603, depending upon the applicant/licensees commitment regarding design criteria. The RTS review should address all topics identified as applicable by Table 7-1. Major design considerations that should be emphasized in the review of the RTS are:
* Design basis
* Single failure criterion
* Quality of components and modules
* Independence
* Defense-in-depth and diversity
* System testing and inoperable surveillance
* Use of digital systems

The same requirements are also applicable to ESFAS instrumentation. NRC noted that the application does not provide information showing how the bypass system capability meets all of the above guidance. Also, the application does not state that Cook has done the failure mode and effects analysis to determine if the failure of bypass system will not have any impact on accident analyses, or would not create new potential accidents. The current DC Cook licensing basis does not include any mass addition transients or accidents. As such, a change in the ESFAS or RTS system that creates the possibility for an inadvertent injection of water into the reactor coolant system (such as the inadvertent start of a charging pump), would require the licensing basis to include mass addition transients in the DC Cook licensing basis. If the licensing basis did not include these transients, neither the licensee nor the NRC could conclude that the proposed change poses no safety concern. There was no discussion on the brand and type of system, or the method used for the qualification of this system.

Summary of Plant Status - January 1 through March 31, 2005

Unit 1 was operated at or near full power during the period until March 26, 2005, when the reactor was shut down for the Cycle 20 refueling outage (U1C20). Unit 1 was shut down in Mode 6 (Refueling) at the end of the period. On January 15, 2005, NRC approved an emergency license amendment to extend the 72-hour allowed outage time of TS 3.5.2 to preclude shutting down Unit 1 until the West charging pump could be restored to an operable status.

Unit 2 was operated at or near full power until January 22, 2005, when the reactor was shut down to troubleshoot and repair a malfunctioning control rod. Following repairs to an electrical cable connector for the control rod and some additional maintenance activities, the reactor was started up and synchronized to the grid on January 23, 2005. Unit 2 operated at or near full power for the remainder of the period.

Unit 2 operated at or near full power until October 2, 2004, when the reactor was shut down for the Cycle 15 refueling outage (U2C15). Following the outage, the unit was synchronized to the grid on November 9, 2004. Unit 2 then operated at or near full power until November 22, 2004, when the reactor was shut down to allow repair of a steam leak on the right moisture separator reheater drain tank and to allow determination of the location of an unidentified reactor coolant system leak, which was found to be from the pressurizer manway cover. Following repairs, the unit was synchronized to the grid on November 29, 2004. Unit 2 then operated at or near full power for the remainder of the period.

Cook - metal fatigue section of license renewal application - draft RAI teleconference summary

Section 4.3.3 of the license renewal application (LRA) discusses Cook's evaluation of the impact of the reactor water environment on the fatigue life of components. The LRA references the fatigue sensitive component locations for an early vintage Westinghouse plant identified in NUREG/CR-6260, "Application of NUREG/CR-5999 Interim Fatigue Curves to Selected Nuclear Power Plant Components". The LRA indicates that the design usage factors provided in Table 5-98 of NUREG/CR-6260 were used for the evaluation of the charging nozzle, safety injection nozzle and RHR tee. The design usage factors were based on an evaluation of the Turkey Point facility, including a plant specific evaluation of the RHR piping and detailed finite element analyses of the charging and safety injection nozzles. NRC's draft Request for Additional Information (RAI) asks Cook to discuss the applicability of these analyses to the D. C. Cook facility. "The discussion should include a comparison of piping sizes and thicknesses, including the design of the thermal sleeves between D. C. Cook and Turkey Point. The discussion should also include a comparison of the number and type of design transients cycles between D. C. Cook and Turkey Point." During a May 6, 2004 teleconference, NRC requested Cook to consider the safety evaluations for Surry and North Anna to aid in the response to this D-RAI. Cook indicated that the question was clear. Cook gets new VP, from NMC

Chris Bakken resigned to return to PSEG earlier this month. American Electric Power has hired Nuclear Management Co. (NMC) Senior VP Mano K. Nazar to replace him. Mr. Nazar will be senior vice president and chief nuclear officer, effective October 1. Before joining NMC, Mr. Nazar worked twenty years for Duke Power, including a stint as plant manager at Oconee. He will report to Bob Powers, AEP's executive vice president - generation. In addition to his Cook duties, Mr. Nazar will be responsible for oversight of AEP's 25% share of South Texas Project. In announcing the hiring, Mr. Powers said that "Mano ... has established a track record as a hands-on leader who can achieve excellence from the people, processes and equipment at a nuclear plant." Cook commits to repairing 'craze cracking' indications on head penetration

Cook plant reported this morning to NRC about cracks discovered in February 2002. The cracks are on a vessel head penetration, and were described by Cook thusly: "... five small, shallow indications ... closely spaced ... initial calculations showed a crack depth of 0.117 inches." The company concluded there was "no through-wall leakage" and "the reactor coolant pressure boundary remains intact."

When the cracking was evaluated in 2002, the plant concluded it was not significant enough to warrant repairs. The cracking has "not shown any significant growth" since then. The plant had the opportunity to check the cracks again because both units are still shut down since the April 24 fish story.

May 5, 2003 - D.C. Cook's application to increase it's rated power output to 1108 MWe was approved. The plant expects to boost output before month's end. This represents an 18 MWe uprate -- about 1.5%. The main basis for the uprate was the increased accuracy of measurements of feedwater flow. The NRC press release notes that the agency's request for public comment published in Federal Register resulted in no public comments.

April 24-25, 2003 - Event Report - Fish story - a bevy of fish at intake screens degraded flow to turbine, prompting manual reactor trip, and to EDGs, prompting all to be declared inoperable. The Alert lasted about 25 hours.
